Dear Patients and Visitors,
Protecting the privacy of our patients' health information is important. We are pleased to tell you that in 1996, Congress passed a law called the Health Insurance Portability and Accountability Act (HIPAA) designed to help protect the privacy of that health information. Part of this law, called the HIPAA Privacy Rule, was implemented in 2003.
The Privacy Rule requires us to give all patients a copy of our Notice of Privacy Practices ("the Notice"). This Notice explains our use of a patient's medical or health information. The rule also requires us to ask patients to sign a form called an acknowledgement. By signing this form the patient confirms that she/he received a copy of the Notice. Patients may download and print a PDF of the acknowledgement below, and bring it with them to their appointment.
